What Does This Test Measure?
Mean platelet volume is the average size of the platelets in a sample. Its value comes from a simple biological fact: young platelets are larger than old ones. When the bone marrow is producing platelets rapidly, it releases large, immature platelets into the circulation and the MPV rises.
That makes MPV a rough indicator of platelet turnover, in much the same way that the reticulocyte count indicates red cell turnover. It is nearly always interpreted alongside the platelet count, because the combination distinguishes a marrow that is failing to produce from one that is producing hard to replace platelets being destroyed elsewhere.
Larger platelets are also more metabolically active and more readily aggregate, which is why a raised MPV has been studied as a cardiovascular risk marker. That association is real but weak, and MPV is not used to guide treatment decisions.

What High MPV Means
High MPV with a low platelet count is the most informative combination. It indicates that the marrow is responding appropriately by releasing large young platelets, so platelets are being consumed or destroyed in the periphery rather than under-produced. Immune thrombocytopenia is the classic example, along with disseminated intravascular coagulation, thrombotic thrombocytopenic purpura, hypersplenism and pre-eclampsia.
High MPV with a normal or raised platelet count is seen in myeloproliferative disorders, and after bleeding or recovery from marrow suppression when production is ramping up. Hyperthyroidism, smoking, obesity and diabetes are associated with modest elevations.
Inherited giant platelet disorders such as Bernard-Soulier syndrome and MYH9-related disease produce strikingly large platelets, sometimes so large that analysers miscount them as red cells and report a spuriously low platelet count.
What Low MPV Means
Low MPV with a low platelet count suggests the marrow is not producing effectively, the opposite conclusion. This pattern is seen with marrow suppression from chemotherapy or radiotherapy, aplastic anaemia, marrow infiltration by cancer, and after certain viral infections. It is a more concerning combination than the high-MPV pattern and generally prompts further marrow assessment.
Low MPV also occurs in Wiskott-Aldrich syndrome, an inherited immunodeficiency characterised by unusually small platelets, and in some chronic inflammatory and autoimmune conditions. A low MPV with a normal platelet count is common, usually unexplained, and rarely of clinical importance on its own.
How to Prepare and What Affects the Result
No fasting or preparation is required, since MPV comes with a routine full blood count. What matters far more is timing between collection and analysis. Platelets swell in EDTA anticoagulant over time, so MPV rises steadily the longer a sample waits, and a result from a delayed specimen can appear abnormal when the blood is entirely normal.
Where MPV is genuinely important, laboratories aim to run the sample within a couple of hours, or use citrate anticoagulant which is more stable. Platelet clumping in EDTA can also cause a falsely low platelet count with a distorted MPV, and this is confirmed by examining a blood film and repeating the count in citrate.
Common Misinterpretations
The most common error is treating MPV as meaningful on its own. Interpreted without the platelet count it says almost nothing; the diagnostic information lies entirely in the combination of the two.
The second is failing to allow for sample age. Because platelets swell in EDTA, a mildly raised MPV in a sample processed late is more likely an artefact than a finding, and repeating on a fresh specimen resolves most such results.
The third is over-reading the cardiovascular association. MPV correlates statistically with cardiovascular risk across populations, but the effect is small, it is not part of any risk calculator, and it should not influence individual treatment decisions.
