What Does This Test Measure?
Prostate-specific antigen circulates in two forms. Most is bound to protein carriers, and a smaller portion circulates unbound, or free. The free PSA test measures that unbound portion, and it is almost always reported as the percent free PSA, free divided by total, expressed as a percentage.
The reason this is useful comes from where the PSA originates. PSA leaking from benign enlarged prostate tissue tends to remain unbound, whereas PSA produced by cancer cells is more likely to be bound to carrier proteins. So a higher proportion of free PSA points toward benign disease, and a lower proportion raises the probability of cancer.
The test exists to solve one specific problem: what to do about a total PSA in the ambiguous 4 to 10 ng/mL range, where most men do not have cancer but some do, and a biopsy carries real risks.
There is no simple normal range, because the result is a probability rather than a threshold. In men with a total PSA between 4 and 10 ng/mL, a percent free PSA above 25 percent is associated with a low probability of cancer, often under 10 percent, while a value below 10 percent is associated with a substantially higher probability, frequently above 50 percent. Values between 10 and 25 percent fall in a graded middle zone. The cut-off used varies between centres and is applied alongside age, prostate size, examination findings and family history.

Free PSA is a second-line test, ordered specifically when a total PSA is borderline, conventionally 4 to 10 ng/mL, and the decision about biopsy is genuinely uncertain. It is used to refine risk rather than to screen.
It is also used in men who have declined or wish to avoid biopsy, in those with an enlarged prostate where benign tissue plausibly explains the PSA, and in monitoring men on active surveillance. It is not helpful when total PSA is very high, where the probability of cancer is already substantial, nor when it is clearly normal.
What High Free PSA Means
A high percent free PSA, above 25 percent, is the reassuring direction. It suggests the PSA is coming from benign prostatic hyperplasia, prostatitis or another non-malignant cause, and is associated with a low probability of finding cancer on biopsy. In many men this is sufficient to justify monitoring rather than proceeding to biopsy.
It does not exclude cancer. A minority of cancers occur in men with a high free fraction, so the result shifts probability rather than settling the question, and it is always weighed alongside examination findings, PSA trend over time, imaging and family history.
What Low Free PSA Means
A low percent free PSA, below 10 percent, indicates that most of the circulating PSA is bound, a pattern more characteristic of malignant tissue. In the 4 to 10 ng/mL total PSA range this is associated with a materially higher probability of cancer and generally supports proceeding to further assessment, usually MRI followed by targeted biopsy.
A low ratio is not a diagnosis. It raises probability, and the confirmation still requires imaging and tissue. Conversely, prostatitis and recent instrumentation can distort both fractions and produce misleading ratios, which is why testing is deferred after infection or catheterisation.
How to Prepare and What Affects the Result
Free PSA is measured on the same sample as total PSA, and both must be run together for the ratio to mean anything. The sample requires careful handling: free PSA is less stable than total PSA and degrades if the specimen sits unseparated, which artificially lowers the ratio and can push a result toward a falsely concerning zone.
Avoid ejaculation for 48 hours, and vigorous cycling or a digital rectal examination shortly before the test. Testing should be deferred for at least six weeks after a urinary infection, prostatitis, catheterisation or prostate biopsy. Finasteride and dutasteride roughly halve total PSA, and this must be accounted for. Biotin supplements interfere with several assays.
Common Misinterpretations
The most common error is applying the ratio outside the range where it was validated. Percent free PSA is intended for total PSA values of roughly 4 to 10 ng/mL; below and above that range it adds little and can mislead.
The second is treating a reassuring ratio as an all-clear. It reduces the probability of cancer but does not eliminate it, and it does not replace examination, PSA trend or imaging.
The third is ignoring sample handling. Because free PSA degrades faster than total, a delayed or poorly processed specimen lowers the ratio and can convert a reassuring result into an apparently worrying one, prompting an unnecessary biopsy.
