What High Folate Means

High folate: almost always from supplementation, benign. What Low Folate Means

Low folate: very common. This masking effect can delay the diagnosis of B12 deficiency by months, during which subacute combined degeneration of the spinal cord may become irreversible. For this reason, a high folate result must always be interpreted alongside a B12 level and, when neurological symptoms are present, with additional markers like methylmalonic acid (MMA) or homocysteine. Simply seeing a normal CBC and high folate does not rule out a B12 crisis.

Another common error is assuming that a normal serum folate rules out folate deficiency. Because serum folate reflects recent intake, a person with chronically poor diet who consumed a folate-rich meal the day before testing may show a normal level while RBC folate stores are depleted. Conversely, low serum folate in the context of acute illness or alcohol binge can be transient and not indicative of chronic deficiency. In both scenarios, the RBC folate provides the more reliable picture. Additionally, biotin interference can produce a falsely high serum folate, leading clinicians to miss a true deficiency.
