Estradiol is the most potent of the three human estrogens and the dominant one during the reproductive years. In women it is produced mainly by the developing ovarian follicle; in men and in postmenopausal women, most of it comes from the conversion of testosterone in fat, muscle and other tissues by the enzyme aromatase.
Its effects reach well beyond reproduction. Estradiol maintains bone density, influences cholesterol handling and vascular function, and acts on brain tissue. This is why the fall at menopause produces effects across many systems rather than reproductive symptoms alone.
In premenopausal women the level changes dramatically across the menstrual cycle, rising through the follicular phase to a sharp peak just before ovulation, falling, then rising again more modestly in the luteal phase. A result cannot be interpreted without knowing the day of the cycle.

In men, it is checked when investigating gynaecomastia, infertility, low libido, or when monitoring testosterone therapy, since a proportion of administered testosterone is converted to estradiol. In children it forms part of the assessment of early or delayed puberty.
What High Estradiol Means
In premenopausal women, a high estradiol most often simply reflects the timing of the sample near ovulation. Genuinely raised levels occur with ovarian cysts, estrogen-producing ovarian tumours, and during fertility treatment, where excessive levels warn of ovarian hyperstimulation syndrome.
In men and postmenopausal women, the commonest cause is increased aromatase activity in adipose tissue, so obesity is the leading explanation. Liver disease raises estradiol because the liver clears it, and alcohol contributes through the same route. Testosterone therapy raises estradiol as a direct consequence of conversion. In men, a persistently high estradiol with breast tissue development warrants assessment for a testicular or adrenal tumour, though these are uncommon.
What Low Estradiol Means
In women, low estradiol with a high FSH means the ovary is no longer responding, the normal picture after menopause, and the finding that defines premature ovarian insufficiency when it occurs before 40. Low estradiol with a low or normal FSH points instead to the pituitary or hypothalamus, seen with excessive exercise, low body weight, eating disorders, chronic illness, high prolactin, or pituitary disease. That distinction determines the entire subsequent workup.
Persistently low estradiol accelerates bone loss regardless of cause, which is the main reason it is treated rather than simply observed in younger women. In men, low estradiol, often from over-suppression with aromatase inhibitors, causes joint discomfort, reduced libido and bone loss, since men need estradiol for bone health too.
How to Prepare and What Affects the Result
Fasting is not required. The critical variable is cycle timing: for baseline assessment the sample is usually taken on day 2 to 5 of the cycle, and the day should always be recorded on the request. For postmenopausal women and men, timing is unimportant but assay sensitivity is, and a method capable of measuring low concentrations accurately should be used.
Hormonal contraception, hormone replacement, fertility drugs, and testosterone therapy all alter results substantially. Biotin supplements interfere with many immunoassays and are best stopped a couple of days beforehand. Topical estrogen gels can contaminate a sample if applied to the arm.
Common Misinterpretations
The dominant error is interpreting a premenopausal result without the cycle day. The same value can be entirely normal or clearly abnormal depending on when it was drawn, and an unlabelled sample is often uninterpretable.
The second is reading estradiol without FSH in a woman with absent periods, since the two together separate ovarian failure from a pituitary or hypothalamic cause and point to completely different investigations.
The third, particularly common in men on testosterone therapy, is treating a mildly raised estradiol as something to eliminate. Estradiol is required for bone density, libido and joint comfort in men, and aggressive suppression with aromatase inhibitors frequently causes more symptoms than the original elevation.
