Urine Protein Blood Test

A comprehensive guide to understanding your urine protein lab result; what it measures, normal ranges, and what high or low levels mean for your health.

What Does This Test Measure?

Urine protein measures all protein appearing in urine, not just albumin. Albumin is usually the largest component, but the total also includes immunoglobulin light chains, tubular proteins such as beta-2 microglobulin, and Tamm-Horsfall protein secreted normally by the kidney.

Like the albumin test, it is reported as a ratio to creatinine, the protein-to-creatinine ratio, which corrects for urine concentration and has largely replaced 24-hour collections.

The distinction from ACR matters more than it first appears. Albumin leak indicates glomerular damage. Total protein also captures tubular disease and, importantly, light chains from multiple myeloma, which contain no albumin at all. A patient with myeloma can have heavy proteinuria with a near-normal ACR, and testing only for albumin would miss it entirely.

Normal Range

Under 150 mg per day, or a protein-to-creatinine ratio below roughly 150 mg/g. Above 3,500 mg per day, or a ratio above 3,500 mg/g, defines nephrotic-range proteinuria, which alongside low serum albumin and oedema constitutes nephrotic syndrome. A urine dipstick reports protein semi-quantitatively from trace to 4+, but it is insensitive to low concentrations and heavily influenced by urine dilution, so a quantitative ratio is used for anything that matters.

Why Your Doctor Ordered This Test

The usual trigger is protein detected on a routine urine dipstick, which then needs quantifying. It is also ordered when investigating oedema, unexplained kidney impairment, or suspected glomerular disease.

It has a specific role where non-albumin protein is suspected: in multiple myeloma and related conditions, where light chains are the relevant protein, and in tubular disorders. It is central to monitoring known kidney disease, to assessing pre-eclampsia in pregnancy, and to evaluating anyone with a large or rising albumin leak where the total burden matters.

What High Urine Protein Means

Persistent proteinuria indicates kidney damage, and the level points toward the type. Glomerular disease produces the heaviest proteinuria (diabetic nephropathy, membranous nephropathy, focal segmental glomerulosclerosis, IgA nephropathy, lupus nephritis and amyloidosis) and nephrotic-range values almost always indicate a glomerular cause.

Tubular disease produces more modest proteinuria, typically under 1 to 2 g per day, from interstitial nephritis, medication toxicity and inherited tubular disorders.

Overflow proteinuria occurs when a small protein is produced in such excess that it overwhelms reabsorption. Multiple myeloma light chains are the key example, and the classic clue is proteinuria substantially exceeding albuminuria; a discrepancy that should prompt serum and urine electrophoresis and free light chain testing.

Transient proteinuria follows fever, vigorous exercise, heart failure and urinary infection. Orthostatic proteinuria, in which protein appears only when upright, is common in adolescents, entirely benign, and confirmed by a normal first-morning sample.

What Low Urine Protein Means

A normal urine protein is the expected result and indicates that neither the glomerular filter nor tubular reabsorption is leaking appreciably.

It is not a complete assessment of kidney function. Someone can have a substantially reduced eGFR with minimal proteinuria, particularly in polycystic kidney disease, obstruction and some vascular kidney disease, so a normal result should never be taken as proof of healthy kidneys on its own.

A normal total protein also does not exclude moderately increased albuminuria. Because albumin can be raised at concentrations below the sensitivity of total protein measurement, someone with early diabetic kidney disease may have a normal protein-to-creatinine ratio and an abnormal ACR. This is precisely why ACR rather than total protein is used for screening in diabetes and hypertension.

How to Prepare and What Affects the Result

A first-morning sample is preferred, both because it is more concentrated and because it excludes orthostatic proteinuria, which resolves overnight. No fasting is required.

Avoid vigorous exercise in the preceding 24 hours. Defer testing during urinary infection, fever, menstruation or acute illness, since all raise protein transiently. Very dilute urine can produce a falsely negative dipstick, and highly alkaline urine a falsely positive one. NSAIDs, ACE inhibitors, ARBs and recent contrast agents alter results and should be declared. Where myeloma is suspected, urine electrophoresis is required in addition, as routine methods detect light chains poorly.

Common Misinterpretations

The most important error is relying on total protein alone in diabetes or hypertension screening. It is not sensitive enough to detect early albuminuria, and a normal result gives false reassurance in exactly the group where early detection changes outcomes. ACR is the correct screening test.

The second is missing the significance of proteinuria that greatly exceeds albuminuria. That discrepancy points to light chains and should trigger investigation for myeloma rather than being treated as ordinary kidney disease.

The third is treating a single dipstick result as diagnostic. Dipsticks vary with urine concentration and pH, and transient proteinuria is common, so any positive finding should be quantified and confirmed before it is acted on.

It is confirmed by a normal first-morning sample following an abnormal daytime one. It requires no treatment and does not progress to kidney disease, but it needs to be distinguished from genuine proteinuria.

What counts as nephrotic-range proteinuria?

More than 3,500 mg per day, or a protein-to-creatinine ratio above roughly 3,500 mg/g. Combined with low serum albumin, oedema and often raised cholesterol, this constitutes nephrotic syndrome. It nearly always indicates glomerular disease and warrants prompt specialist assessment, frequently including a kidney biopsy.

Can a dipstick miss significant protein?

Yes. Dipsticks are insensitive at low concentrations and are strongly affected by how dilute the urine is, so a dilute sample can read negative despite meaningful protein loss. They also detect light chains poorly. Any clinically important question should be answered with a quantitative protein-to-creatinine ratio or ACR rather than a dipstick.
