What Does This Test Measure?
Haptoglobin is a protein made by the liver whose job is to mop up free haemoglobin. When a red blood cell breaks apart inside the circulation, the haemoglobin it releases is toxic to the kidneys and a useful source of iron for bacteria. Haptoglobin binds it immediately, and the complex is cleared by the liver.
That clearance mechanism is what makes the test useful. Each molecule of haptoglobin is consumed when it binds haemoglobin, so a falling haptoglobin is a direct footprint of red cell destruction. It is among the most sensitive markers of intravascular haemolysis available.
There is one important complication. Haptoglobin is also an acute-phase reactant, meaning the liver produces more of it during inflammation. So inflammation pushes it up at the same time haemolysis pushes it down, and the two can cancel out, which is why it is read alongside CRP.
Roughly 30–200 mg/dL, with meaningful variation between laboratories. Newborns have very low or absent haptoglobin for the first few months of life, which is normal and not a sign of haemolysis. Around one in a thousand people of European descent, and considerably more in some African populations, have congenital absence of haptoglobin; a benign inherited state that produces a permanently undetectable result.

The main reason is suspected haemolysis: unexplained anaemia, jaundice with a raised indirect bilirubin, dark urine, or a rising reticulocyte count without evident bleeding.
It is used to investigate autoimmune haemolytic anaemia, transfusion reactions, mechanical destruction from prosthetic heart valves, and inherited conditions such as G6PD deficiency and hereditary spherocytosis. It is also monitored in thrombotic microangiopathies and in paroxysmal nocturnal haemoglobinuria. Occasionally it is used in the other direction, as a general marker of inflammation.
What High Haptoglobin Means
A raised haptoglobin is usually a sign of inflammation rather than a problem in its own right. As an acute-phase protein it rises with infection, tissue injury, surgery, burns, malignancy, and inflammatory conditions such as rheumatoid arthritis and inflammatory bowel disease. Corticosteroid treatment raises it, as do androgens.
It also rises with biliary obstruction and nephrotic syndrome. On its own, a high value adds little that CRP does not already convey.
The clinically important consequence of a high haptoglobin is what it can conceal. If someone has both inflammation and haemolysis, the inflammatory rise can lift a haemolysis-depleted haptoglobin back into the normal range, producing a falsely reassuring result. Interpreting haptoglobin alongside CRP, LDH and reticulocytes avoids this trap.
What Low Haptoglobin Means
A low haptoglobin is the finding that matters, and in the right context it is strong evidence of haemolysis. It is most sensitive for intravascular destruction, where red cells rupture inside blood vessels, and haptoglobin is often undetectable in severe cases. Causes include autoimmune haemolytic anaemia, transfusion reactions, mechanical destruction by prosthetic valves, microangiopathic processes such as thrombotic thrombocytopenic purpura and haemolytic uraemic syndrome, G6PD deficiency triggered by drugs or infection, and paroxysmal nocturnal haemoglobinuria.
Extravascular haemolysis, where cells are removed by the spleen, lowers haptoglobin less dramatically and sometimes not at all.
Not every low result means haemolysis. Severe liver disease reduces production, since the liver makes it. Congenital absence produces a permanently undetectable level in a healthy person. Dilution from large-volume transfusion or fluid resuscitation lowers it mechanically.
How to Prepare and What Affects the Result
Fasting is not required and no special preparation is needed. The most important practical issue is sample quality: haemolysis occurring in the tube during a difficult blood draw releases haemoglobin that consumes haptoglobin in vitro, producing a falsely low result. A traumatic venepuncture is a recognised cause of spurious haemolysis findings, and laboratories flag visibly haemolysed samples for this reason.
Haptoglobin should ideally be measured alongside LDH, indirect bilirubin, reticulocytes and CRP, since these together establish whether haemolysis is genuinely present. Corticosteroids, androgens and estrogen-containing medication alter levels and should be declared.
Common Misinterpretations
The most important error is accepting a normal haptoglobin as evidence against haemolysis in someone who is also inflamed. The acute-phase rise can mask genuine consumption entirely, and a normal value in an unwell patient does not exclude red cell destruction.
The second is diagnosing haemolysis from a low haptoglobin alone. Liver disease, congenital absence, dilution and a difficult blood draw all produce low values without any red cell destruction, which is why the full panel is needed.
The third is expecting haptoglobin to detect all haemolysis. It is far more sensitive to intravascular destruction than to the extravascular kind, so a normal result does not rule out splenic removal of red cells.
