What Does This Test Measure?
This is a calculated ratio rather than a measured substance, dividing blood urea nitrogen by creatinine. Its usefulness comes from the fact that the kidney handles these two waste products differently.
Creatinine is filtered by the glomerulus and then largely left alone; very little is reabsorbed. It therefore tracks filtration rate fairly faithfully. Urea is also filtered, but a variable proportion is reabsorbed back into the body, and crucially that reabsorption increases when the body is conserving water.
So when someone is dehydrated or has reduced blood flow to the kidney, urea is reabsorbed avidly while creatinine is not, and the ratio climbs. That single behaviour is what the ratio detects: it distinguishes a kidney that is under-perfused but structurally intact from one that is genuinely damaged.
Roughly 10:1 to 20:1 when using BUN and creatinine in conventional US units. A ratio above 20:1 suggests a pre-renal cause such as dehydration or gastrointestinal bleeding, while a ratio below 10:1 suggests low protein intake, liver disease or a urea-wasting state. Note that outside the US, urea is often reported in mmol/L rather than as BUN, and the ratio is calculated differently with quite different reference figures.

It helps in assessing acute kidney injury, distinguishing dehydration from intrinsic kidney damage, investigating suspected gastrointestinal bleeding, and evaluating heart failure or liver disease where kidney perfusion is often reduced. It also has a role in nutritional assessment, where a persistently low ratio suggests inadequate protein intake.
What High BUN/Creatinine Ratio Means
A high ratio, above roughly 20:1, points to a pre-renal problem; the kidney is structurally fine but not receiving enough blood, so it conserves water and reabsorbs urea.
Dehydration is the commonest cause, from poor intake, vomiting, diarrhoea, diuretics or excessive heat. Reduced effective circulation from heart failure, cirrhosis, sepsis or blood loss produces the same picture.
Gastrointestinal bleeding is an important and frequently missed cause. Blood in the gut is digested protein, which is absorbed and converted to urea, raising BUN without any change in creatinine. A rising ratio in someone with anaemia should prompt consideration of an occult bleed.
A high protein diet, corticosteroid treatment and catabolic states such as major trauma, burns or severe infection all increase urea production. Urinary tract obstruction raises both values but often the ratio too.
What Low BUN/Creatinine Ratio Means
A low ratio, below about 10:1, generally means urea is disproportionately low rather than creatinine being high.
Liver disease is a key cause, because urea is synthesised in the liver; in advanced cirrhosis, urea production falls and the ratio drops, which can mask kidney impairment. Low protein intake, malnutrition and starvation reduce urea production for the same reason.
Overhydration and the syndrome of inappropriate antidiuretic hormone secretion dilute urea. Pregnancy lowers it through increased filtration and plasma volume. Dialysis removes urea efficiently and lowers the ratio.
A low ratio can also arise from a raised creatinine rather than a low urea, in someone with substantial muscle mass, after rhabdomyolysis, or with creatine supplementation. This is worth recognising, because it reflects muscle rather than kidney disease.
How to Prepare and What Affects the Result
Fasting is often requested because glucose is on the same panel, and a high protein meal shortly before testing can modestly raise urea. Hydration status is the single biggest influence on the result, and being significantly dehydrated at the time of the draw will raise the ratio independently of any disease.
Recent heavy exercise, creatine supplements and high-protein diets raise the values and should be declared, as should corticosteroids and diuretics. Because the ratio depends on both components, it should always be interpreted alongside the absolute BUN and creatinine values and, ideally, an eGFR.
Common Misinterpretations
The most common error is using the ratio in isolation. A normal ratio with both values markedly raised still indicates significant kidney impairment, and a high ratio in someone with entirely normal absolute values usually means nothing at all.
The second is forgetting gastrointestinal bleeding. A raised ratio is a recognised early clue to an occult upper GI bleed, particularly alongside a falling haemoglobin, and attributing it purely to dehydration can delay the diagnosis.
The third is applying US reference figures to results reported in SI units. Outside the US, urea is usually reported in mmol/L rather than as BUN in mg/dL, and the conventional 20:1 threshold does not apply.
